If you or a household member is experiencing a serious short-term medical condition that significantly affects health, daily activities, or treatment needs, you may qualify for temporary protections designed to help you maintain service during your recovery.
This protection may be right for you if:

What you'll need
- A Serious Illness Protection form (pdf) completed by a Licensed Physician (LP), Medical Doctor (MD), Doctor of Osteopathic Medicine (DO), Nurse Practitioner (NP), Physician Assistant (PA), or Registered Nurse (RN)
Protection lasts for an initial 21 days and may be extended if approved.
How to apply
You can apply for these protections online, by mail, or by fax.Online
- Complete the Serious Illness Protection form (pdf)
- Visit our secure file transfer portal
- Select Upload and attach your completed documents
- Enter your email address in the From field
- Enter “AccountProtections” in the To field
- Enter your service address in the Subject field
- Include your full name and phone number in the Message field
- Select Drop Files Off to submit
